This booking form constitutes a legally binding agreement. Payment must be received in full prior to the event. Conference details will be issued subject to receipt of payment. The CMA cannot be held responsible for the non-arrival of registration information.
If you have not heard from us within 7 days prior to the conference, please contact the conference administrator. Cancellations, confirmed in writing two weeks before the conference date, will be refunded minus a £20 administration charge. We regret no refund can be made after that period, for whatever reason, although substitution will be accepted if notified in writing prior to the event.

All information and data that is processed by The Cardiomyopathy Association is in accordance with the provisions of the Data Protection Act 1998. Your details will only be used for administrative purposes by Cardiomyopathy Association staff or agents and will not be released to any third party without your explicit and written permission.
